Step two

Your journey to a straight smile begins

What happens at your third appointment and beyond:

  • Your aligners will arrive approximately three weeks after ordering
  • You will start wearing your aligners and change them every 1-2 weeks
  • The aligners will gently move your teeth in small, safe increments. Initially, you'll return every 2 to 4 weeks. This will change to every 6 to 8 weeks for check-ups and to receive additional aligners.
  • Upon completing your treatment, you will receive removable and/or fixed retainers to preserve and safeguard your new beautiful smilefor life.

Consultation

Your first dental implant consultation visit will be with our principal dentist Dr Alpa Johar and will last for approximately 1 hour.

This appointment includes:

  • Examination of your mouth, teeth, gums and your bite.
  • X-rays
  • Mouth cancer screening
  • Understand why you want implants and discuss alternatives, so you are fully aware of your options.

We approach your mouth with a holistic approach to dentistry and will give you info about all the ins and outs of dental implants including advice on supplements you may want to take to make your bones stronger and strengthen your immunity.

This consultation is refundable if you carry on with the implant placement. You will receive an estimate of what the treatment will cost.

Step 2

Treatment planning

We will always take a CBCT Scan for the planning phase of dental implants. The Scan takes approximately 15 seconds and you will see the result immediately.

This 3D xray provides detailed information about your bone anatomy, information about important structures such as your sinus or major nerves.

This scan allows us to accurately assess the bone you have in your jaws and to see the exact space and measurements for where the proposed implant/s are to be placed safely.

  • Intra oral scan: series of photos taken inside your mouth to make a digital model of your
    teeth and soft tissues.
  • Surgical guide for precision: We combine CBCT 3D scan with the intra oral scan which allows us to make a surgical guide for predictable and accurate placement of the dental implant.
  • Photography/videography: To plan the aesthetics of your smile. Patients find this step interesting as they learn a lot about their mouth!

Step 3

Bespoke treatment plan with exact cost

  • Transparent and honest, Dr Alpa Johar will spend time studying your scan and digital study models to accurately assess what your options are for treatment. She will create your individual plan with information specific to your mouth based on the consultation and scans. Depending on the complexity of your situation, you will be given a detailed bespoke treatment plan within 1-7 days.
  • Consent to start treatment: When you are ready and have all your questions answered, including finance options, you will sign the treatment plan and a time slot will be allocated for your implant placement appointment.

Steps 2 and 3 can sometimes be combined depending on complexity of the treatment

Step 4

Treatment starts

Phase 1: Placement of the dental implant

Surgical placement of the implant. For a single implant, the actual procedure takes around 1 hour. For multiple implants it takes longer. The implants are inserted into the jaw generally below the bone level.

At the end of this implant placement appointment you will have a few stitches. These are removed after 10-14
days.

The healing stage takes 3-4 months. This healing process is called osseointegration. These implants are then left for a period of time to osseointegrate prior to teeth being attached

After around 4 months, you return to check if the implant has integrated sufficiently. We use the ISQ instrument to confirm that we are ready to start making the permanent restoration.

Phase 2: Placing the final restoration

A scan is taken of the mouth to allow the manufacture of the tooth/teeth/denture. The decision of what will be made will have been discussed before the implant will have even been placed.

With this information, the technicians will begin to fabricate whatever has been planned. The lab takes approx. 3 weeks to make the restoration. Shortly after the scan, we can place the permanent restoration to complete your smile!

I have a loose denture. Can dental implants help me?

Conventional dentures are often uncomfortable and, due to the movement, limit a person’s ability to speak and eat normally. Because of this, many people feel self-conscious wearing their dentures. The dentures can be secured into place by locking on to 2-4 implants. Implants are commonly used to replace individual missing teeth, but they can also be used to secure or stabilise loose dentures in either the upper or lower jaws. Instead of having a crown (cap) fitted to the top of the implant a precision-made attachment is fitted which inserts into the underneath surface of the denture, securing the denture to the implant. In the lower jaw this can be achieved with as few as two implants. If there is sufficient bone available and you are willing to have more implants inserted then you can often be fitted with a completely solid set of teeth attached to the dental implants (a full-arch bridge). This set of teeth would stay in permanently and would look and feel very similar to a natural set of teeth.

Is dental implant placement painful?

Dental implants are typically placed under local anaesthetic to ensure you are comfortable throughout the treatment. If you are very nervous or suffer from dental phobia we can offer you sedation. Why Dr Alpa Johar chose implant dentistry

"During my undergraduate dentistry degree, I was still learning, still finding my confidence, and still discovering what kind of dentist I wanted to become. It was a busy, demanding time filled with lectures, clinics, and late nights of studying.

One term, my great aunty came to visit me. I hadn’t seen her in a while, and I was excited for her trip. She had always been a strong, warm presence in our lives - the kind of person who made every room feel safe just by being in it.

But when she arrived, something was different.

One evening while we were talking, I noticed her eyes filling with tears. At first, she tried to brush it off, but eventually she told me what was wrong. Her dentures hurt so much that she could barely eat. Family meals, which she used to love, had become painful and embarrassing. She had to remove the dentures to eat her food.

She felt frustrated and embarrassed. She wanted her dignity back. Something as simple as being able to eat with her loved ones again is all she was asking for.

Hearing this while I was studying dentistry hit me deeply. Here I was, learning about smiles, oral health, and restoring confidence, yet my dear grandaunt was suffering right in front of me.

I wanted to fix it immediately, to take away her pain, but I was still a student.

I didn’t yet have the full skills or authority to treat her myself.

In that moment, I felt a mix of helplessness and determination. Helpless because I couldn’t solve her problem right away. Determined because I knew this was exactly why I had chosen this path. Seeing my great aunt in tears over something as fundamental as eating changed how I viewed dentistry. It was no longer just exams, procedures, or textbooks. It became personal. It became about dignity, comfort, and quality of life.

I wasn’t a qualified dentist yet. I couldn’t be her solution at that moment.But that day became the reason I promised myself I would become the kind of dentist who could be.

I went on to graduate in 1997 from the prestigious Faculty of Odontology at the University of Oslo,Norway. Since then, I have dedicated my career to continually refining and expanding my skills across all aspects of dentistry, with a particular passion for implant dentistry.

My postgraduate training includes advanced implant education from the Cambridge Academy of Dental Implantology and the Trinon Collegium Practicum in Brazil, experiences that have shaped my clinical philosophy and strengthened my commitment to excellence.

Over the years, dentistry has become more than a profession - it has become a family legacy. Both of my children chose to follow in my foot steps and are now dentists themselves.
